Application forms will be kept in a secure place with restricted access to maintain confidentiality of information.
Applicants will be contacted when new or existing physicians are taking new patients.
The Stakeholders Group is a partnership of the Township of Centre Wellington, Groves Memorial Community Hospital, Wellington-Dufferin-Guelph Health Unit, Centre Wellington District High School, the Centre Wellington Chamber of Commerce, the Township of Guelph-Eramosa, the Township of East Garafraxa, local physicians and interested community members.
